Maternal Child Health Case Manager
Nuvance Health has a network of convenient hospital and outpatient locations - Danbury Hospital, New Milford Hospital, Norwalk Hospital and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center and Vassar Brothers Medical Center in New York - plus multiple primary and specialty care physician practices locations, including The Heart Center, a leading provider of cardiology care, and two urgent care offices.
Non-acute care is offered through various affiliates, including the Thompson House for rehabilitation and skilled nursing services, and the Home Care organizations.
SummaryThis position functions as an integral member of an interdisciplinary care team, the goal of which is to proactively identify and address biopsychosocial needs of perinatal patients and their families.
With a primary focus on the underserved, people of color, as well as the high-risk perinatal population, this position performs culturally competent, comprehensive assessments and interventions, in collaboration with the health care team and community providers and resources.
Responsibilities Performs psychosocial assessments and develops care plans to meet clients' needs.
Interventions may include:
Education (i.
e.
, self-management skill development, community resources), Referrals to community resources, Mandated reports to the Department of Children and Families, Assistance with transitional care needs (i.
e.
, follow up appointments), Short term counseling and crisis intervention, Assistance with concrete tasks such as applications for financial assistance and other programs.
Establishes professional, effective relationships with key community stakeholders and resources.
Assists clients in addressing gaps/ deficits identified in social determinants of health.
Maintains lists of current community resources and demonstrates working knowledge of assistance provided by these agencies, as well as eligibility requirements and application processes.
Demonstrates agility in being able to move between different settings in the community and hospital to ensure optimal services to targeted populations.
Demonstrates resourcefulness and creativity in problem solving as well as the ability to prioritize work.
Attends case conferences and other relevant meetings in the hospital and community.
Documents in the electronic medical record.
Collects relevant data and tracks and trends issues as appropriate.
Evaluates processes and protocols in place against the goals of the position and proactively initiates necessary changes in order to ensure goal attainment.

Degrees in related fields with eqiuvalent prior experience with this population may be considered in lieu of educational requirement.
Prior experience working in a maternal child health field/position preferred.
Oral and written English and Spanish proficiency required.
Demonstrates cultural competence Ability to function autonomously Valid CT drivers' license required.
